Job summary

Angels Care Home Health is seeking a Registered Nurse Case Manager to coordinate comprehensive home health care. You will conduct admission visits, assess patient eligibility, and develop individualized plans of care in collaboration with the treating physicians and care team.

Qualifications

• Graduate of an accredited nursing program or school of nursing.
• Current state license as a Registered Nurse.
• Valid driver’s license and reliable transportation.

Responsibilities

• Coordinate all aspects of the patient’s home care as the RN Case Manager.
• Perform admission visits to evaluate eligibility for home health services.
• Develop and implement the plan of care with input from relevant medical disciplines.
• Collaborate with interdisciplinary teams to ensure quality patient outcomes.
