Job summary

Home Sweet Home Healthcare is seeking a compassionate Registered Nurse (RN) to join our home health team in Maricopa County, AZ. The role involves traveling to patients’ homes to provide skilled nursing, wound care, and medication management, with a focus on independence and quality of life.

This PRN/Home Health RN opportunity offers flexible scheduling, autonomy in clinical decisions, and meaningful one-on-one patient care in patients’ homes, supporting families across East Valley and beyond.

Qualifications

• Current Arizona RN license in good standing.
• Graduate of an accredited nursing program; BSN preferred.
• Minimum of 1 year of RN experience; home health preferred.
• Experience with geriatric patients and in-home care.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive nursing assessments in patients’ homes.
• Deliver skilled nursing care including wound care, medication management, treatments, and patient monitoring.
• Develop, implement, and update individualized home health care plans.
• Educate patients and caregivers on disease management, medications, safety and ongoing care needs.
• Collaborate with physicians, case managers, therapists, and interdisciplinary care teams.
• Complete accurate and timely clinical documentation in compliance with agency standards.
• Monitor patient progress and communicate changes in condition to the care team.
• Support improved patient outcomes, independence, and quality of life.
