Job summary

Home Sweet Home Healthcare is seeking a compassionate Registered Nurse (RN) to join our Home Health team in Arizona. As a Home Health RN, you will provide skilled nursing services across Maricopa County, visiting patients in their homes and coordinating care with physicians and families.

The role offers autonomy in the field, a schedule from Monday through Friday, and opportunities to serve geriatric and medically complex patients while maintaining work-life balance.

Qualifications

• Current Arizona RN license in good standing is required.
• Graduate of an accredited nursing program; BSN preferred.
• Minimum of 1 year RN experience; Home Health experience preferred.

Responsibilities

• Conduct comprehensive nursing assessments in patients' homes.
• Deliver skilled nursing care including wound care and medications.
• Develop and update individualized home health care plans.
• Educate patients and caregivers on disease management and safety.
• Collaborate with physicians, case managers, and therapists.
• Complete accurate clinical documentation per standards.
• Monitor patient progress and communicate changes to the care team.
• Support improved patient outcomes and quality of life.

Home Health RN Responsibilities

As a Home Health Registered Nurse (RN), you will provide skilled nursing services throughout Maricopa County and surrounding Arizona communities. Responsibilities include:

• Conduct comprehensive nursing assessments in patients’ homes
• Deliver skilled nursing care including wound care, medication management, treatments, and patient monitoring
• Develop, implement, and update individualized home health care plans
• Educate patients and caregivers on disease management, medications, safety and ongoing care needs
• Collaborate with physicians, case managers, therapists, and interdisciplinary care teams
• Complete accurate and timely clinical documentation in compliance with agency and regulatory standards
• Monitor patient progress and communicate changes in condition to the care team
• Support improved patient outcomes, independence, and quality of life

Qualifications

To be considered for this RN Home Health position in Arizona, candidates should meet the following requirements:

• Current Arizona Registered Nurse (RN) license in good standing
• Graduate of an accredited nursing program (BSN preferred)
• Minimum of 1 year of RN experience required
• Home Health nursing experience strongly preferred
• Experience working with geriatric patient populations preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and organizational abilities
• Valid driver’s license, reliable transportation, and ability to travel throughout the East Valley, Scottsdale, and North Phoenix areas
• Ability to work independently while delivering compassionate mobile care
