OSF HealthCare is seeking an RN Case Manager for Home Health to oversee patient care from infancy to geriatric, coordinating nursing, therapy, social work, home care aides, respiratory therapy, equipment and infusion services at the patient’s residence.

The role focuses on optimizing outcomes, identifying high-risk patients, and ensuring evidence-based, patient-centered plans of care with OASIS impact. Licensed RNs in IL or MI with BLS are required.

Qualifications

• Education: Completion of nursing program
• Experience: 2+ years as RN in healthcare
• Licensure/ Certification: RN License in IL or MI; BLS; valid driver’s license

Responsabilités

• Identify, assess, plan, implement, monitor and evaluate patient care
• Collaborate with providers and interdisciplinary teams
• Coordinate services across home health, therapy, social work, and equipment
• Ensure documentation accuracy (OASIS) and outcomes for quality measures
