Job summary

OSF HealthCare is seeking an RN Case Manager for Home Health to coordinate care for patients in their homes, performing assessments and guiding interdisciplinary teams to deliver comprehensive, evidence-based care. Qualifications

• Education: Completion of nursing program.
• Experience: RN in healthcare for two years or more.
• Licensure/Certification: RN license in IL or MI depending on state.
• Current AHA HealthCare Provider BLS is required.
• Valid state driver's license with active auto insurance.
• Strong interpersonal and communication skills.
• Solid computer skills, including Microsoft Office.
• Analytical and problem-solving abilities with attention to detail.
• Maintains own transportation and auto insurance; adheres to driving laws when on duty.

Responsibilities

• Identify, assess, plan, implement, monitor and evaluate clinical outcomes for caseload patients.
• Coordinate inter-disciplinary services with provider, nursing, therapy, social work, and home care staff.
• Ensure high-risk patients are identified and focused on to reduce hospitalizations.
• Provide evidence-based, patient-centered care at patients' residences with effective use of OASIS documentation.

Pay range for this position is $37.05 - $54.54/hour. Actual pay is based on years of licensure. This is an Hourly position.

OSF will invest in you day 1! External candidates with 5+ years of direct experience will receive a bonus of 24 hours of Vacation/Holiday time frontloaded on day 1. This is in addition to our generous Paid Time Off plans!

Overview

POSITION SUMMARY: The RN Case Manager - Home Health is responsible for identifying, assessing, planning, implementing, monitoring and evaluating clinical outcomes, service utilization, and resource management for patients on their caseload. The Case Manager is responsible for a scope of care that encompasses all ages from infancy through geriatric and demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. The Case Manager collaborates with the provider to coordinate inter-disciplinary services which may include nursing, therapy, social work, home care aide, respiratory therapy, home medical equipment and home infusion services. The Case Manager is accountable for attaining the best possible patient outcomes including identification and focused oversight of patients at high risk for hospitalization. The Case Manager provides evidence-based, patient-centered care at the patient’s place of residence, using critical thinking and decision-making skills. The Case Manager has a thorough understanding of OASIS documentation and plan of care development, and their impact on 5 Star Agency Performance and Value Based Purchasing performance.

Qualifications

REQUIRED QUALIFICATIONS

Education: Completion of nursing program

Experience: Experience as RN in a healthcare environment for two years or more.

Licensure/ Certification: RN License in IL or MI depending on state they are working in.

Current American Heart Association HealthCare Provider BLS is required prior to start date of employment.

Valid State Driver's License with proof of active auto insurance.

Other Skills/ Knowledge: Excellent interpersonal and communication skills.

Solid computer skills, including proficiency with Microsoft software.

Strong analytical and problem-solving skills, with the ability to be detail oriented.

Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty.

Maintains a valid driver's license.

Preferred Qualifications

Education: BSN

Experience: Experience as an RN in Home Health, OASIS and Case Management experience.

Licensure/ Certification: OASIS Certification

Other Skills/ Knowledge: Electronic Medical Record and Microsoft application basic proficiency
