Job summary

OSF HealthCare is seeking an RN Case Manager for Home Health to identify, assess, plan, implement, monitor, and evaluate clinical outcomes for patients from infancy to geriatric. You will coordinate with a multidisciplinary team to deliver patient-centered care at the patient’s home, using strong clinical judgment and OASIS knowledge.

Requires RN license in IL or MI, BLS, and a valid driver's license. Familiarity with EMR and Microsoft tools is beneficial, with BSN preferred.

Qualifications

• Education: Completion of nursing program.
• Experience: RN in healthcare environment for two years or more.
• Licensure/ Certification: RN License in IL or MI depending on state.
• BLS: American Heart Association HealthCare Provider BLS required prior to start date.
• Valid driver's license with active auto insurance.
• Excellent interpersonal and communication skills.
• Solid computer skills, including Microsoft Office applications.
• Strong analytical and problem-solving skills.

Responsibilities

• Identify, assess, plan, implement, monitor, and evaluate clinical outcomes and service utilization for caseload.
• Coordinate inter-disciplinary services with providers, including nursing, therapy, social work, and home care.
• Ensure high-risk patients receive focused oversight to optimize outcomes.
• Apply knowledge of OASIS documentation and plan of care development.
