Job summary

OSF HealthCare is seeking a Registered Nurse Home Care with responsibilities spanning pediatric to geriatric patients. The RN Case Manager coordinates inter-disciplinary care and delivers patient-centered services throughout the home health episode.

Qualified candidates hold active nursing licensure in Illinois or Michigan, current BLS, and a valid driver's license. A BSN is preferred; own transportation is required to support visit schedules and care delivery.

Qualifications

• Licensure by State Board of Nursing (IL or MI)
• AHA HealthCare Provider BLS required
• Own transportation and auto insurance; safe driving on duty
• Valid driver's license must be maintained

Responsibilities

• Coordinate inter-disciplinary care for patient-focused outcomes.
• Make care decisions regarding home health episode of care.
• Provide patient-centered care with critical-thinking for all ages.
• Oversee care coordination, visit patterns, and episode monitoring.
• Manage resources and ensure appropriate episode management.
