Qualifications

• Current Driver's License, vehicle insurance, and access to dependable vehicle or public transportation

Responsibilities

• Supervise and coordinate clinical services for home health patients.
• Coordinate interdisciplinary team and ensure adherence to physician plan of care.
• Manage patient evaluations, SOC visits, and documentation of care plans.
• Oversee patient safety, care quality, and regulatory compliance.
• Communicate with physicians, clinicians, and families to adjust plans as needed.
