Job summary

Alternate Solutions Health Network is seeking an experienced Registered Nurse Case Manager to lead patient assessment, care planning and coordination for home health patients in the Kettering area.

You will supervise care delivered by nurses and aides, perform OASIS assessments, update orders, and educate patients and families while ensuring compliance with state and federal guidelines. A valid RN license and strong collaboration with physicians are essential.

Qualifications

• Minimum of one year of RN experience; home health preferred.
• Strong communication with patients, families, physicians and staff.
• Calm demeanor, patient-centric and compassionate.
• Knowledge of nursing best practices and clinical decision-making.
• Ability to prioritize, multi-task and meet deadlines.
• Able to work independently and as part of a team.

Responsibilities

• Oversee patient care from start to end of episode.
• Update doctor orders in patient chart as needed.
• Notify physician of significant changes in patient condition.
• Coordinate multi-disciplinary care and supervise staff.
• Provide skilled nursing care in patient homes as ordered.
• Document care, assessments and education in a timely manner.
• Complete OASIS assessments and develop care plans.
• Adhere to Medicare/agency guidelines and maintain licensure.
