Redcrest Home Health is seeking a Registered Nurse Case Manager (RNCM) to lead patient assessments, plan of care, and coordination across disciplines in the Indianapolis area. You will supervise care, document accurately, and ensure compliance with state and federal regulations while maintaining a patient-first approach.

The role requires at least one year of RN experience in acute care, preferably with home health, strong communication, and a valid driver's license.

Qualifications

• Minimum of one year of RN experience in an acute care setting.
• Home health experience is desired.
• Ability to effectively communicate with patients, families, physicians and co-workers.
• Compassionate and caring while working with patients.
• Knowledgeable on nursing best practices and able to make appropriate nursing judgments.
• Detail-oriented with strong prioritization and time-management.
• Valid driver’s license and auto insurance in your name.

Responsabilités

• Oversees the overall care of the patient from beginning to end of episode.
• Updates all doctor orders in patient chart accordingly.
• Notifies doctor of any significant changes in patient’s condition.
• Manages multi-disciplinary care as applicable.
• Provides skilled nursing care in the patient’s home as ordered by the attending physician.
• Observes and monitors patient conditions.
• Performs assessments to develop an individualized plan of care and makes adjustments as needs change.
• Administers medication as prescribed by the physician.
• Promotes continuity of care with appropriate admissions, transfers and discharges.
• Counsels patient and family on disease/injury processes and how to manage.
