Redcrest Home Health is seeking an experienced Registered Nurse Case Manager to lead assessment, plan of care, and coordination of multi-disciplinary services in patients' homes in the Indianapolis area.

The RNCM will educate patients and families, document thoroughly, and ensure compliance with Medicare and company policies while maintaining an active RN license. On-call rotation and travel within the territory are required.

Qualificações

• Minimum of one year of RN experience in acute care.
• Home health experience is desired.
• Strong communication and patient education skills.

Responsabilidades

• Oversees patient care from start to end of episode.
• Updates doctor orders in patient chart.
• Notifies physician of significant changes in condition.
• Delivers skilled nursing care per physician orders.
• Counsels patient and family on disease and management.
• Completes clinical documentation per Medicare/agency guidelines.
• Maintains license and participates in on-call rotations.
