Job summary

Advocate Health in Wisconsin seeks a full-time nurse in Milwaukee north/southern Ozaukee county area. The role focuses on conducting patient visits, updating care plans, and collaborating with an interdisciplinary team.

The position requires an Associate’s Degree in Nursing, 2 years’ nursing experience, and an active RN license with BLS. 40 hours/week, weekday schedule, and local travel with a company car are included.

Qualifications

• Proficient clinical nursing skills with ability to work under supervision and make sound judgments.
• Excellent written and verbal communication and interpersonal abilities.
• Ability to build rapport with patients, staff and physicians.
• Demonstrated ability to make critical, independent decisions.
• Good organization, prioritization and problem-solving skills.

Responsibilities

• Manage all activities related to assigned visits and communicate changes to the primary RN.
• Assess patient needs, plan of care, and implement prescribed interventions.
• Perform scheduled visits and collaborate with interdisciplinary team as needed.
• Assess patient response to treatments and update care plans accordingly.
• Schedule patients and case conferences with the primary RN.
• Assist discharge planning and ensure continuity of care after discharge.
• Document and update patient records per guidelines.
