UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care. Under the supervision of an RN or MD, collaborate with a Care Advocate and Social Worker to ensure appropriate care levels.

The role offers a comprehensive benefits package and competitive total cash compensation, with pay components per visit point and hourly rates.

Qualifications

• Current RN license (state(s) you are assigned)
• Valid driver's license, reliable transportation and liability insurance
• Current CPR certification
• 1+ years of clinical RN experience

Responsabilités

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care
• Makes referrals to other disciplines as needed and documents rationale
• Promotes patient health and independence through teaching and self-care techniques
• Supervises LPNs and HHAs
• Completes documentation timely and accurately
• Regularly communicates patient progress to the clinical manager and care team
• Plans and provides staff education
• Performs on-call responsibilities and services to patients/families
• Participates in clinical development and continuing education
• Other duties as assigned
