Job summary

UnitedHealth Group in Oregon seeks a Nurse Case Manager to guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care.

Under the supervision of an RN or MD, you will help ensure patients receive the appropriate level of care; provide education, coaching, and treatment-decision support; and collaborate with a Care Advocate and Social Worker.

Qualifications

• Current RN license valid for the state(s) assigned.
• Current CPR certification.
• 1+ year of clinical RN experience.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Perform patient assessments and develop care plans with the care team.
• Refer to other disciplines as needed and document rationale.
• Promote health and independence through teaching and rehabilitation.
• Supervise LPNs and HHAs.
• Complete timely and accurate documentation.
• Communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Participate in on-call duties and after-hours coverage.
• Engage in clinical development and continuing education.
• Perform other duties as assigned.
