Job summary

Amedisys is hiring a Registered Nurse Case Manager for full-time day shifts to deliver home health services. The role includes patient assessments, care-planning, and collaboration with the care team to promote independence and healing at home.

Eligible candidates have 1+ year RN experience, an active RN license in the relevant state, CPR certification, and a valid driver's license with insurance. A $10,000 sign-on bonus is offered along with competitive benefits.

Qualifications

• 1+ year RN clinical experience; VP approval if less than 1 year
• Current RN license for assigned state(s)
• CPR certification
• Valid driver's license, reliable transportation and liability insurance

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care
• Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so
• Promotes patient health and independence through teaching and rehabilitative measures
• Supervises LPNs and HHAs
• Completes documentation timely and accurately
• Regularly communicates patient progress to the clinical manager and care team
• Plans and provides staff education
• Performs on-call responsibilities and on-call services to patients/families
• Participates in clinical development and continuing education
