Job summary

Amedisys is seeking a registered nurse case manager for full-time days to support patients in their homes. You will perform patient assessments, collaborate with the care team, and develop and implement plans of care that promote healing where patients feel most comfortable.

Key duties include making referrals, teaching self-care, supervising LPNs and HHAs, completing documentation on time, and communicating patient progress to the clinical manager.

Qualifications

• Minimum 1 year of clinical RN experience.
• Current state RN license.
• Current CPR certification.
• Valid driver's license and liability insurance.

Responsibilities

• Perform patient assessments and develop care plans with the care team.
• Make referrals as indicated and document rationale.
• Educate patients to promote self-care and independence.
• Supervise LPNs and HHAs.
• Ensure timely and accurate documentation.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform on-call responsibilities and on-call services to patients/families as assigned.
• Participate in clinical development and continuing education programs.
• Other duties as assigned.
