Job summary

Amedisys is seeking an experienced Registered Nurse Case Manager for full-time, Monday–Friday, 8am–5pm, in Homewood, AL. Manage patient plans of care, coordinate with the care team, and supervise LPNs and HHAs while ensuring accurate documentation.

Earn $73,000–$80,000 annually during orientation, then switch to a per-point pay method. Company benefits include health plans, PTO, wellness rewards, retirement plan, and mileage reimbursement.

Qualifications

• 1+ year of clinical RN experience; must be approved by VP Clinical if less than 1 year.
• Current RN license for the assigned state(s).
• Active CPR certification and valid driver's license with vehicle; liability insurance.

Responsibilities

• Performs patient assessments with care team to plan and implement care.
• Make referrals and document rationale for services.
• Teach and support self-care to promote independence.
• Supervise LPNs and HHAs as assigned.
• Document timely and accurately; communicate progress to clinical manager.
