550 EHS Home Health Care Services, Inc. seeks a Registered Nurse for a full-time home health role serving Sugar Grove, Yorkville, Plano, Bristol, Big Rock and nearby areas.

M-F, 8am-5pm, with weekend/holiday rotations; on-call 3–4 times a month; sign-on bonus up to $3,000. Responsibilities include establishing therapeutic nurse-patient relationships, coordinating care, ensuring high-quality, cost-effective services, and educating patients and families.

Qualifikationen

• Graduate from an accredited school of nursing.
• Earn 10 continuing education credits annually.
• One year of medical surgical experience.
• Current Illinois RN license.

Aufgaben

• Develops and maintains a therapeutic nurse/patient relationship throughout the care continuum.
• Assists with planning and implementing individualized care, seeking consultation as needed.
• Ensures quality, cost-effective care and supports patient safety and regulatory requirements.
• Leads and participates in education for patients, families, and department staff.
• Coordinates referrals, discharge planning, and interdisciplinary care across teams.
