Safe Life Home Health Care, LLC is seeking a Home Health Registered Nurse to manage patient care in patients' homes in the Chicago area. The role emphasizes comprehensive assessments, medication administration, and collaboration with interdisciplinary teams to prevent hospitalization and promote recovery.

The ideal candidate has at least 1 year of home health experience, is proficient with WellSky Kinnser EMR, and holds an Illinois RN license with CPR certification.

Qualifications

• Minimum 1 year of home health experience.
• Proficiency with WellSky Kinnser EMR required.
• Valid Illinois RN license and CPR certification.
• Valid driver's license and auto insurance.

Responsibilities

• Provide strong clinical skills and patient-centered care.
• Adhere to established policies and procedures.
• Conduct comprehensive home health assessments.
• Monitor patient status and report changes to physicians.
• Develop and implement individualized care plans.
• Administer medications and treatments per orders.
• Document progress and outcomes accurately.
• Educate patients and families on disease management and safety.
• Coordinate with interdisciplinary teams for discharge planning and referrals.
• Maintain medical records per regulatory standards.
• Respond to patient inquiries with guidance and support.
• Attend staff meetings and care conferences.
