Safe Life Home Health Care, LLC is seeking a Home Health Registered Nurse Case Manager in Melrose Park, IL. This role requires a minimum of 1 year home health experience and proficiency in Well Sky Kinnser EMR to coordinate patient care in the home setting.
You will assess patients, develop care plans, manage medications, and collaborate with interdisciplinary teams to prevent hospitalizations while ensuring high-quality, compassionate care.
Competenze
- Illinois RN license valid.
- CPR certification valid.
- Valid driver’s license.
- Auto insurance valid.
- Minimum 1 year home health experience.
- Proficiency in Well Sky Kinnser EMR.
- Strong clinical assessment and communication skills.
- Compassionate and patient-centered approach.
Mansioni
- Provide strong clinical skills and patient-centered care.
- Adhere to established policies and procedures.
- Conduct comprehensive home health assessments.
- Monitor patient status and report changes to physician/RN.
- Develop and implement individualized care plans.
