Job summary

• Graduate of accredited school of nursing with active RN license.
• Two years minimum nursing experience; home health/hospice preferred.
• Ability to document clearly and manage care plans.

Responsibilities

• Organizes and directs provision of services to assigned patients.
• Develops and updates plans of care with the care team.
• Documents in EMR at point of care and keeps records current.
• Participates in on-call, weekly case conferences and QAPI program.
• Maintains visit schedules and ensures quality patient outcomes.
