Bethesda Health Group is seeking an RN to organize, coordinate and direct ongoing home health services for assigned patients. You will ensure quality patient care, supervise staff as needed, and maintain up-to-date plans of care with input from patients, families and physicians.

Responsibilities include comprehensive assessments, drug regimen reviews, and timely documentation in the EMR. You will participate in on-call schedules, weekly case conferences, and the agency’s QAPI program while

Formación

• RN Missouri license in good standing
• Two years of varied professional nurse experience, preferably in home health/hospice
• Strong communication and documentation skills
• Ability to develop and update the plan of care with patients and families
• Ability to lift/move patients 10–200+lbs with assistance as needed

Responsabilidades

• Organizes, coordinates and directs ongoing services for assigned patients
• Ensures delivery of quality patient care and supervises nursing staff as required
• Develops and updates the plan of care with input from patients, families and physicians
• Documents at the point of care in the EMR to keep accurate patient records
• Maintains an accurate visit schedule and participates in on-call and QAPI activities
• Performs required physician summaries at SOC, Recert, Transfer and discharge
