Job summary

Carilion Clinic seeks an RN Case Manager - Home Health to assess and coordinate care for patients in the Lexington region. The role emphasizes care planning, documentation, and collaboration with a multidisciplinary team to prevent hospitalizations and improve outcomes.

Responsibilities include performing OASIS visits as needed, educating patients and families, and ensuring timely care adjustments while maintaining quality and safety standards, in a fast-paced home setting.

Qualifications

• Graduate of accredited Registered Nurse Program.
• Strong critical thinking skills and clinical assessment abilities.
• Demonstrates knowledge of life span growth and development.

Responsibilities

• Assess, plan, implement, and evaluate home health patient care to achieve optimal outcomes.
• Collaborate with the health care team to develop an interdisciplinary plan of care.
• Administer and document treatments and medications using electronic records.
• Coordinate visits, adjust plans as patient status changes, and ensure safety.
• Supervise LPNs or aides as required and support ongoing education.
