Job summary

Amedisys, Inc. is seeking a Registered Nurse Case Manager to support our home health and hospice services in Virginia. The role centers on coordinating patient-focused plans and delivering care at home, working with the clinical team to promote recovery and independence.

You will assess patients, teach self-care, supervise LPNs and HHAs, ensure accurate documentation, and participate in ongoing clinical development. A valid RN license and CPR certification are required.

Qualifications

• 1+ year of clinical RN experience; approval by VP Clinical if under 1 year.
• Current RN license for the assigned state(s).
• Current CPR certification is required.
• Valid driver’s license with reliable transportation and liability insurance.

Responsibilities

• Perform patient assessments and collaborate on care plans.
• Refer to other disciplines as needed and document rationale.
• Promote health and independence through teaching and rehab measures.
• Supervise LPNs and HHAs.
• Complete timely, accurate documentation.
• Communicate patient progress to clinical manager and team.
• Plan and provide staff education.
• On-call responsibilities for patients/families as assigned.
• Engage in clinical development and continuing education.
• Perform other duties as assigned.
