Job summary

AMEDISYS HOME HEALTH is seeking a Registered Nurse Case Manager to join our team and support patients at home. You will assess patients, collaborate with the care team to create and implement a plan of care, and coordinate referrals as indicated.

You will also educate patients for self-care and supervise LPNs and HHAs, with timely documentation and regular progress updates. This role requires 1+ year RN experience, a current state RN license, CPR certification, and a valid driver’s license with

Qualifications

• 1+ year of clinical RN experience.
• Current RN license in the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• Note - if less than 6 months RN experience, RN Intern program required.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services.
• Participates in clinical development and continuing education.
• Other duties as assigned.
