Job summary

Amedisys, part of the Optum family, is seeking a Nurse Case Manager to guide patients and families through the care continuum. Under RN or MD supervision, you will assess needs, coordinate care, support discharge planning and manage ongoing outpatient care.

You will educate, coach, and collaborate with a Care Advocate and Social Worker, while supervising LPNs and HHAs and contributing to clinical development and performance improvement initiatives.

Qualifications

• Current RN license, state-specific as needed.
• Current CPR certification.
• 1+ years of clinical experience as an RN.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicate patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
