Job summary
Amedisys is seeking a registered nurse case manager in Alabama for a full-time role that supports patients at home. You will perform assessments, develop care plans, supervise LPNs and HHAs, and coordinate with the clinical team. On-call weekend rotation is required, with orientation pay transitioning to per-visit compensation.

Qualifications
- 1+ year of clinical RN experience.
- Current RN license for assigned state(s).
- Current CPR certification.
Responsibilities
- Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
- Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
- Promotes patient health and independence through teaching and rehabilitative measures.
- Supervises LPNs and HHAs.
- Completes documentation timely and accurately.
- Communicates patient progress to the clinical manager and care team.
- Plans and provides staff education.
- Performs on-call responsibilities as assigned.
- Participates in clinical development and continuing education.
