Job summary

Mass General Brigham Home Care, Inc. is seeking a full-time Registered Nurse to join the Tanner City team, primarily in Topsfield and Boxford, with broader support across listed locations.

As a case-managed home-care RN, you will deliver a patient-centered plan of care, collaborate with LPNs, therapists and social workers, and perform about 5–6 visits daily with time built in for documentation and coordination. Benefits include tuition reimbursement.

Qualifications

• Graduate of an approved School of Nursing.
• Current Massachusetts license as Registered Nurse.
• Minimum of one-year previous nursing experience in acute care setting.
• Travel within MGB Healthcare at Home geographic area.
• Ability to work with various computer software required and knowledge of hand-held patient care computer devices preferred.
• Strong interpersonal and customer service and ability to work with a diverse population.
• Ability to communicate effectively in writing, verbally and electronically.
• Ability to work as a member of an interdisciplinary team of health care providers.
• Ability to organize and prioritize work, and adapt to changing situations.
• Ability to work independently, be self-directed and adapt to unpredictable circumstances.

Responsibilities

• Serve as case managers, overseeing the full plan of care while collaborating with LPNs, therapists, and social workers.
• Typically complete 5-6 visits per day, Monday through Friday, with time for documentation, chart review, and coordination built into the schedule.
