Job summary

Mass General Brigham Home Care is seeking a part-time RN to join the Minuteman team. The role covers Framingham and Waltham with additional territories supported by the wider team.

You will deliver skilled nursing in patients' homes, serving as a case manager and collaborating with an interdisciplinary team to ensure high-quality care. Applicants should be a Graduate of an approved School of Nursing and hold MA RN licensure, with at least one year of acute care experience; home care preferred.

Qualifications

• Graduate of an approved School of Nursing.
• Current Massachusetts license as Registered Nurse.
• Minimum of one year previous nursing experience in acute care; prior home care experience preferred.
• Travel required within MGB Healthcare at Home geographic area.
• Ability to work with various computer software and hand-held patient care devices.
• Strong interpersonal and customer service and ability to work with a diverse population.
• Ability to communicate effectively in writing, verbally and electronically.
• Ability to organize and prioritize work, and adapt to changing situations.
• Ability to work independently, be self-directed and adapt to unpredictable circumstances.

Responsibilities

• Serve as case manager, overseeing the full plan of care while collaborating with LPNs, therapists, and social workers.
• Complete approximately 5-6 visits per day, with time for documentation, chart review, and coordination.
• Provide quality clinical services to patients in home settings and ensure adherence to policies.
