Job summary

Mass General Brigham Home Care, Inc. is seeking a skilled registered nurse to join the Start of Care North team. You will assess patients, develop care plans, and provide skilled nursing in patients’ homes while collaborating with an interdisciplinary team.

You’ll manage your schedule with autonomy within a supportive group. The full-time role (36 hours), hybrid work type, is based in Massachusetts with travel across the MGB Home Care geographic area.

Qualifications

• Graduate of an approved School of Nursing.
• Massachusetts Registered Nurse license.
• At least one year of acute care nursing experience (home care experience preferred).
• Comfort working with various computer systems; handheld clinical devices preferred.
• Strong interpersonal and customer service skills with diverse populations.
• Excellent communication skills—written, verbal, and electronic.
• Strong organization and prioritization skills with the ability to adapt in a dynamic environment.
• Ability to work independently and remain self-directed in unpredictable situations.
• Ability to collaborate effectively as part of an interdisciplinary care team.
• Willingness to travel within the MGB Healthcare at Home geographic area.

Responsibilities

• Complete patient assessments and admissions.
• Develop and manage individualized care plans.
• Provide skilled nursing care, education, and treatment.
• Coordinate care with patients, families, and providers.
• Document care and review charts.
• Collaborate with the interdisciplinary team.
• Deliver high-quality, patient-centered care.
