What You’ll Do

• Serve as a case manager overseeing the full plan of care.
• Collaborate closely with LPNs, therapists, and social workers.
• Complete 5‑6 patient visits per day, Monday through Friday.
• Provide quality clinical services in various community settings.
• Perform admissions process and case management responsibilities.
• Utilize thorough and timely electronic documentation.
• Deliver skilled nursing care including assessment, teaching, and treatment.
• Demonstrate high‑quality customer service and financial awareness.

Requirements

• Education: Graduate of an approved School of Nursing
• Experience:
  • Minimum of one year previous nursing experience in an acute care setting
  • Prior home care experience preferred
• Licenses & Certifications: Current Massachusetts license as Registered Nurse
