Angels Care Home Health in Texas is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of the patient’s home care. You will interact with other medical disciplines as needed and provide professional care to the patient, including admission visits and determining eligibility for home care services.

Qualifications include a nursing degree, current RN license, valid driver’s license, CPR proof, hepatitis consent/declination, and reliable transportation.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.

Responsabilités

• Coordinates all aspects of the patient’s home care.
• Interacts with other medical disciplines as needed.
• Performs the admission visit to the patient in the home and determines eligibility for home care services.
• Develops the plan of care to be followed.
