Job summary

Angels Care Home Health is seeking an RN Case Manager to coordinate patient home care, collaborate with medical teams, and assess eligibility and planning for services. You will conduct admission visits in the patient’s home and implement the care plan.

The role requires nursing licensure, a valid driver's license, and CPR certification, with preference for prior RN experience. We offer comprehensive benefits and a supportive, growth-focused environment.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse, Current state Driver’s License.
• One-year experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines and provide professional care.
• Perform admission visit to the home and determine eligibility for home care services; develop plan of care.
