Job summary

Interim HealthCare Inc. in Bloomington, IN is seeking a Registered Nurse Case Manager to deliver high-quality, compassionate home health care. You will conduct assessments, coordinate care, and support patients and families in managing their plans.

The role emphasizes collaboration with caregivers, LPNs, RNs, and providers, with flexible full-time scheduling, a sign-on bonus, and daily pay options to support work-life balance. A valid Indiana RN license and CPR/BLS are required.

Qualifications

• Active RN license in Indiana
• Current CPR/AED/BLS/First Aid certification
• 1 year of RN experience in home health or similar setting
• OASIS experience preferred
• Practical trach and/or ventilator experience preferred, not required

Responsibilities

• Conduct in-person patient interviews and comprehensive physical assessments.
• Oversee the implementation and ongoing assessment of the patient's plan of care through the management of home health aides, LPNs, RNs, and other caregivers.
• Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.
• Provide education to patients and families on proper home health care procedures (e.g., wound care, IV administration, medication management).
• Work to decrease readmissions by promoting preventative care and ensuring continuity of care.
