Job summary

Carle Health is seeking an experienced Home Care RN/Field Case Manager to provide skilled nursing visits across Champaign and surrounding counties. You will work on an interdisciplinary home care team and coordinate care plans with patients and families.

You will perform comprehensive case management, conduct OASIS/485 changes, and ensure timely documentation. The role offers autonomy with 24/7 clinical backup and competitive pay with mileage reimbursement.

Qualifications

• IL RN license required.
• BLS certification within 30 days.
• Driver's license and proof auto insurance.
• College Diploma in Nursing.
• 1 year nursing experience.

Responsibilities

• Demonstrates understanding of Medicare Home Care benefits and regulatory documentation.
• Performs case management: assessment, planning, coordination, monitoring, implementation and evaluation.
• Interacts with clients, caregivers and families to assess and plan care, arrange services and educate.
• Completes admission documentation within policy and ensures timely transmission of orders and notes.
• Supervises Home Health Aide and coordinates the health care team to maintain continuity of care.
