Job summary
Carle Health is seeking a Home Care RN/Field Case Manager to join our interdisciplinary home health team serving Champaign, Vermilion, and surrounding counties. The role supports coordinated care plans, wound care, IV therapy, and complex nursing tasks in patients' homes.
You’ll work with an experienced team, enjoy autonomy with flexible scheduling, and receive a competitive hourly wage with mileage reimbursement. External candidates must meet licensure and certification requirements.
Qualifications
- IL RN license required.
- BLS certification within 30 days of start date.
- Valid driver’s license and proof of auto insurance.
- Education: College Diploma Nursing.
- Work Experience: 1 year nursing experience.
Responsibilities
- Coordinate and provide direct and indirect patient care per plan of care.
- Perform OASIS assessments for admissions, recertifications, resumptions and discharges.
- Supervise Home Health Aide and coordinate care among the health care team.
- Communicate with MD and care coordinators about patient updates and condition changes.
- Ensure documentation complies with regulatory standards and policy timelines.
