Job summary

Carle Health in central Illinois is seeking an experienced Home Health RN to join the Weekend Program as a Field Case Manager serving the Champaign region. You will work independently and with an interdisciplinary team to coordinate care for patients throughout several counties.

Responsibilities include case management, comprehensive nursing assessments, and education of patients and families, with 12-hour shifts on weekends and on-call support.

Qualifications

• IL RN certification/license is required.
• BLS certification within 30 days of hire.
• Valid driver's license and proof of auto insurance.
• College diploma in nursing required.
• One year nursing experience preferred.

Responsibilities

• Coordinate care plans and provide skilled nursing assessments.
• Act as the case manager within the interdisciplinary home care team.
• Educate patients and families to promote independence and wellness.
• Complete admission and ongoing documentation per policy and OASIS changes.
• Manage orders, visits, and coordination with physicians and care team.
