You will manage IVs, wound care, and medication protocols while collaborating with a multidisciplinary team to deliver holistic care in diverse living environments.

Qualifications

• Associate's degree in nursing (ADN) or BSN required.
• Home Health experience and documentation familiarity highly desired.
• Ability to work autonomously or with a medical/therapeutic team.
• Solid understanding of Home Health regulations and best practices.
• Empathy and versatility to operate in diverse living conditions.

Responsibilities

• Comprehensive patient assessment covering physical, psychological, and social aspects.
• Curate a personalized care plan for each patient and ensure adherence.
• Educate patients or caregivers on essential self-care skills for home management of chronic conditions.
• Oversee medication protocols and management.
• Specialized care including wound and IV management, PICC Line, G-Tube, and Foley care.
• Collaborate with physicians, therapists, and a diverse team to ensure integrated care.
