Elara Caring is hiring a Home Health RN Case Manager in the Arlington/Belmont area of Massachusetts. This full-time role offers a guaranteed salary of $90,000–$115,000 plus a productivity bonus, mileage reimbursement, and comprehensive benefits.

You will guide patients from assessment to discharge with skilled nursing visits, care plan management, and caregiver education, while coordinating with a clinical team and physicians to promote independence at home.

Qualificações

• Massachusetts RN license required.
• Minimum 1 year of clinical nursing experience.
• Ability to travel throughout assigned territory.

Responsabilidades

• Provide skilled nursing visits in patients' homes.
• Develop and update care plans for assigned caseload.
• Coordinate with physicians and other clinicians.
• Teach patients and caregivers how to manage medications and conditions at home.
