Elara Caring is seeking a Home Health RN Case Manager in Massachusetts to support patients across Attleboro, Foxboro, Walpole, Sharon and Easton. You will guide patients from assessment to discharge, perform skilled nursing visits, and coordinate with physicians to ensure seamless care.

The role requires an active MA RN license and at least one year of nursing experience, with strong assessment, communication, and time management skills, plus the ability to travel.

Qualificações

• Current, unrestricted Massachusetts RN license.
• At least one year of clinical nursing experience.
• Strong assessment, communication, and time management skills.
• Ability to work independently and travel throughout the assigned territory.
• Valid driver’s license, current auto insurance, and reliable transportation.
• Ability to sit, stand, bend, lift, and move intermittently and lift 50–100 pounds.

Responsabilidades

• Guide patients through home health care from assessment to discharge.
• Provide skilled nursing visits, develop and update care plans, manage an assigned caseload, and coordinate with physicians and other clinicians.
• Teach patients and caregivers how to manage medications, treatments, and health conditions at home.
