Job summary

Amedisys is seeking a registered nurse case manager to provide in-home care coordination across the service area. You will assess patients, develop care plans with the team, and refer to other disciplines as needed.

Responsibilities include supervising LPNs and HHAs, timely documentation, and regular progress updates to the clinical manager. On-call duties and continuing education are part of the role.

Qualifications

• 1+ year of clinical RN experience.
• Current RN license for the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver’s license with reliable transportation and liability insurance.

Responsibilities

• Perform patient assessments with the care team to develop and implement a plan of care.
• Make referrals to other disciplines as indicated by patient needs.
• Promote patient health and independence through teaching and rehabilitative measures.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
• Regularly communicate patient progress to the clinical manager and care team.
• Provide staff education and participate in on-call responsibilities.
