Job summary

Iowa Home Care is seeking an experienced Registered Nurse Case Manager to coordinate comprehensive home health services across Polk County. You will assess patient needs, create individualized care plans, monitor progress, and maintain clear communication with patients, families, and the clinical team.

The role emphasizes clinical autonomy, organization, and strong documentation. Candidates should have RN licensure, CPR, and a track record in acute care with a passion for home health care.

Qualifications

• Graduate of an accredited nursing program.
• Current RN licensure and CPR certification.
• Excellent observation, verbal and written communication skills.
• Problem solving and math skills; nursing skills per competency checklist.
• Licensed driver with reliable insured vehicle.

Responsibilities

• Coordinate each patient's plan of care from admission through ongoing services.
• Complete start-of-care, resumption-of-care, recertification, and ongoing assessments.
• Evaluate factors affecting the patient’s ability to remain safely at home.
• Develop individualized plans of care based on needs and orders.
• Provide skilled nursing services including medications, treatments, and disease management.
• Monitor progress and adjust the plan as needed; communicate with physicians and healthcare team.
• Educate patients and caregivers about medications, safety, and home care strategies.
• Maintain thorough, accurate clinical documentation.
