Job summary

Iowa Home Care is seeking a Registered Nurse Case Manager to serve patients statewide in Marion County and surrounding areas. The role combines clinical nursing with care coordination, patient education, and advocacy to ensure the right services at the right time.

You will build relationships with patients and families while collaborating with physicians, therapists, aides, and other professionals to support safe, effective home-based care.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN licensure and CPR certification.
• Excellent observation, verbal and written communication skills.

Responsibilities

• Perform comprehensive nursing assessments at admission and throughout the episode of care.
• Identify patient and family needs and establish goals and interventions.
• Create, implement, and update individualized plans of care.
• Provide skilled nursing treatments within scope of practice.
• Coordinate nursing, therapy, and home health services.
• Educate patients and caregivers on medications, disease management, and self-care.
• Maintain accurate clinical documentation and patient records.
• Communicate changes in condition promptly to the care team.
