Job summary

Adara Home Health is seeking an RN Case Manager to provide direct nursing care and oversee home health services in clients’ homes. You will collaborate with clients, families, and the health team to develop and implement goal-centered plans while ensuring quality and financial considerations are met.

The role requires Minnesota RN licensure, a valid driver’s license, and willingness to travel. A strong emphasis on documentation, education, and clinical leadership is essential.

Qualifications

• Licensed RN in Minnesota with no restrictions.
• Valid driver’s license and access to a car.
• Pass initial and ongoing background checks.
• US Citizenship orAlien Work Permit evidence.

Responsibilities

• Provide direct nursing care and oversee home health services.
• Coordinate care with client, family, and health team.
• Maintain accurate documentation meeting payer/billing requirements.
