Job summary

Adara Home Health, Inc. is seeking a Physical Therapist Case Manager to provide and oversee home health services for clients in the Rochester, MN area, traveling to clients' homes for 1:1 care during office hours.

You will collaborate with the client, family, and health care team to develop goal-centered plans, ensure documentation, and provide education. The role includes clinical oversight, scheduling flexibility, and a base salary with productivity and benefits.

Qualifications

• Licensed PT in Minnesota with no restrictions.
• Valid driver’s license and reliable vehicle.
• Ability to travel to client homes daily.
• Background checks and health screenings compliant.
• Demonstrated ability to work independently.

Responsibilities

• Reflects the company’s mission and maintains professional relationships.
• Performs initial and periodic assessments; updates care plans.
• Collaborates with physicians and home health team; updates plans every 60 days.
• Provides client advocacy; ensures safety and reports vulnerabilities.
• Maintains accurate, timely documentation per state and payer rules.
• Educates clients, families, caregivers, and staff.
• Provides direct care per Minnesota PT Act and coordinates with others.
• Oversees licensed clinicians and unlicensed personnel.
• Manages financial aspects of home health services.
• Maintains flexible schedule and adapts to changes.
