Job summary

Adara Home Health seeks a Physical Therapy Case Manager to deliver and supervise home health services in the Minneapolis area. The role requires building client-centered plans, coordinating with families and the health care team, and ensuring care aligns with Minnesota PT practice acts.

You will travel to client homes for 1:1 care, manage a flexible schedule within office hours, and report to the Clinical Supervisor. A strong focus on safety, documentation, and quality outcomes is essential.

Qualifications

• Licensed Physical Therapist in Minnesota (active, not revoked or suspended).
• Strong critical thinking and clinical judgment to assess and plan patient care.
• Excellent communication with clients, families, and care team.

Responsibilities

• Provide and oversee home health services per plan of care.
• Collaborate with client, family, and healthcare team to implement care plans.
• Ensure documentation is accurate, timely, and compliant with state and payer requirements.
