Job summary

Haven Home Health & Hospice in Springfield, MO is seeking a Home Health Registered Nurse (RN) Case Manager. You will provide skilled nursing care in patients' homes and guide each plan of care from start to finish, with a focus on dignity and quality.

Qualifications

Active Missouri RN license in good standing.
Home health RN experience preferred.
Strong clinical judgment and documentation skills.
Ability to manage a flexible schedule including admissions.
Comfort with technology and electronic medical records.

Responsibilities

Assess, plan, implement, and evaluate individualized patient care.
Develop care plans in collaboration with physicians and the interdisciplinary team.
Complete admissions and ongoing nursing assessments.
Educate and support patients and families through changing needs.
Document care accurately and in a timely manner.
