You will assess, plan, implement, and evaluate patient care in collaboration with physicians and an interdisciplinary team, while maintaining thorough documentation and timely admissions.

Qualifications

• RN license in Missouri is required.
• Experience in home health nursing is preferred.
• Strong clinical judgment and accurate documentation are essential.
• Ability to manage a flexible schedule including admissions.
• Familiarity with electronic medical records.

Responsibilities

• Assess, plan, implement, and evaluate individualized patient care.
• Develop care plans with physicians and interdisplinary team.
• Complete admissions and ongoing nursing assessments.
• Educate and support patients and families through changing needs.
• Document care accurately and in a timely manner.
