Responsibilities
- Manage caseload including supervision of LPN visits.
- Engage in routine visits and evaluations for patient interaction.
- Educate patients on health conditions and management.
- Communicate with physicians for orders and care plans.
- Collaborate with physical, occupational, and speech therapists.
