Safe Life Home Health Care, LLC is seeking a skilled Home Health RN Case Manager with a minimum of 1 year home health experience and proficiency in WellSky Kinnser EMR to join our 4-star team. Prioritize patient health and help prevent hospitalization while providing exceptional care in the comfort of patients' homes.

Qualifications

Valid Illinois RN license.
Valid CPR Certification.
Valid Driver's License.
Valid Auto Insurance.
Minimum 1 year home health experience.
Proficiency in WellSky Kinnser EMR.
Strong clinical assessment and communication skills.
Compassionate and patient-centered approach.

Responsibilities

Provide strong clinical skills and patient-centered care.
Understands and adheres to established policies and procedures.
Conduct comprehensive assessments of patients' health status and needs in their home environment.
Observes signs and symptoms and reports to the physician and RN on reactions to treatments.
Develop and implement individualized care plans based on patient assessments.
Administer medications and treatments as prescribed, ensuring safety.
Monitor patients’ progress and document changes in condition and response to treatment.
Educate patients and families on disease process, medications, pain management, fall prevention, infection control, diet/exercise, when to contact healthcare providers.
Collaborate with interdisciplinary teams to coordinate care and referrals.
Maintain accurate medical records in compliance with standards.
Respond promptly to patient inquiries or concerns.
Attend staff meetings and in-services.
